    • Explanation of Grant Request  
      • Dollar amount
      • What is the community impact?
  2. Project description  
    • 2 Bids per items
  3. Copy of 501©3 from the Internal Revenue Service
  4. Organization’s Financial information  
    • Balance Sheet
    • Income Statement
  5. Organization’s Budget
  6. List of Board of Directors

Application must be submitted by the following dates:

  • January 1
  • April 1
  • July 1
  • October 1
